Staff

 Kyle Thompson, Administrator

Kyle joined Azalea Trail Nursing and Rehab in August 2011. Kyle graduated from Grand Saline High School in 2008 and from Texas A&M University in August 2011 and holds a Bachelor of Science in Human Resource Development. In 2012 he obtained his Nursing Facility Administrator’s License. In his spare time, Kyle enjoys playing golf, watching sports, hunting, fishing, and spending time with his family.

About his position with Azalea Trail Nursing & Rehab he says: “The residents make this the most enjoyable job I have ever held. I feel extremely blessed to be a part of this tight knit family. “

 Kay Sanders, Director of Nurses

Kay joined the Azalea Trail team as Director of Nurses in December 2010. Kay is responsible for the nursing and direct care staff. She has been a registered nurse for 18 years receiving her degree at Southern Arkansas University and she moved to Texas in 1996. Most of her nursing career has been working with geriatrics. Kay feels extremely blessed to be a part of the Azalea Trail Nursing team. She enjoys spending her time with her husband of 36 years, three daughters, three son-in-laws, four grandchildren, and real excited about a new granddaughter on the way.

 Marilyn Thompson, Business Office Manager

Marilyn came to work for Azalea Trail Nursing & Rehab in 2006 as the Business Office Manager. Prior to coming to Long Term Care Marilyn worked as the office manager in a doctors office. Marilyn has lived in Grand Saline all her life. She has one son and enjoys spending time with her family.

 Charlotte Skiles, Asst. Director of Nurses/MDS Coordinator

Charlotte has been in nursing for 10 years and loves her job. She has worked at Azalea Trail for 5 of those years. “It’s hard to find caring people like the staff at Azalea. She has 2 daughters and a husband. In her spare time she enjoys horseback riding, playing the piano and spending time with her family.

 Kari Baxter, Activity Director

Kari was born in Odessa, Texas and grew up in Terrell, Texas. After graduating from high school, she went back to West Texas.

Before becoming an Activity Director, Kari volunteered in various nursing homes while taking Activity Classes. She then worked 2 years at a 60 bed Alzheimer's facility in West Texas, before joining the Azalea Trail Nursing and Rehabilitation Center team in 2012.

About her job, Kari says she has great passion for working with people living with Alzheimer's Disease and she has had special training and been to several workshops on Alzheimer's Disease. She also attended extensive workshops with Action Pact, a company that trains in Nursing Home Culture Change.

Kari has been a trainer in Person-First, a program to promote resident-centered care. She is relatively new to Azalea Trail but has already seen the extraordinary care, concern, and love shown to the Residents here.

Kari has one son, Danny and his wife, Tish.

 Cristy A. Lowry, Social Services Director

Cristy was born in Juarez-Mexico, where she attended the University of Juarez and obtained a Bachelor’s Degree in Social Work. She moved to the USA later and in 2001 she became a U.S. citizen. Cristy attended the Texas A&M-Commerce where she obtained a Bachelor’s Degree in Social Work and a Master’s Degree in Social Work. Cristy has a vast experience working in Long Term Care Facilities as a social worker and as a counselor; Cristy is familiar with diverse cultural needs. Cristy is an active member of CASA, NASW, and NACSW. Cristy is bilingual speaking both English and Spanish.

 Peggy Boyles, Medical Records

Peggy has enjoyed working at Azalea Trail in the medical records department since March 2007. Before coming to Azalea Trail she worked for a home health agency. Peggy is married and has two sons. She has lived her all her life in Grand Saline, TX.

 Isabel Casillas, Dietary Service Manager

Isabel was born and raised in Mexico and moved to Texas in 1981. She lived in West Texas for eight years and moved to Grand Saline in 1989. Isabel started working at Azalea Trail in 2006. She has been married for 32 years and has 4 children. Isabel enjoys working at Azalea Trail.

 Wallie Kroontje, Maintenance Director

Wally was born and raised in Michigan. He moved to Texas in 1979. He has been living in Grand Saline, Texas since 1997. He is the father of two daughters and one son. He has 4 grandchildren. He has been With Azalea Trail Nursing and Rehab since 2002. Wally feels blessed to be a part of Azalea Trail, and enjoys his co-workers and taking care of the residents.

 Tamara Housewright, Housekeeping Supervisor

Tamara was raised in Fruitvale, Texas and attended Grand Saline High School. After working 10 years at Mary Kay Cosmetics Manufacturing and 7 years at Garrett Metal Detectors she returned to Van Zandt County and recently joined our team at Azalea Trail Nursing and Rehabilitation Center.

Tamara's hobbies are reading and ancestry. She has 3 children and will soon have her first grand baby. About her job, Tamara says she enjoys it very much and feels very blessed to be part of the Azalea Trail Nursing and Rehabilitation Center team.
